- In the THIRD PARTY BOX – fill in as – NATIONAL FREIGHT MGT WANTIRNA / VIC 3152

- In the CHARGE TO box - SELECT the FREIGHT CHARGES to be paid by NATIONAL FREIGHT
MGT ( NFM )
- Write ACCOUNT NUMBER of NATIONAL FREIGHT MGT. This will be given only to our CUSTOMERS
and is NOT to be used without our CONSENT.
- SERVICE required is to be INDICATED by placing a X in the APPROPRIATE box. If NO
box is ticked, the SHIPMENT will travel as ECONOMY ROAD.
- DESCRIPTION of GOODS ( eg box, carton, skid, pallet )
- NUMBER of ITEM(S). This must match the number of items available for COLLECTION
- The DIMENSIONS of the ITEMS must be in CENTIMETRES ( CM )
- The WEIGHT of the ITEMS must be in TONS
- All Consignments MUST be SIGNED & DATED. In doing so you are AGREEING to the terms
& conditions – which are on the back of every consignment note.
Q R NATIONAL CONSIGNMENT
How to Fill Consignment Note?
- WRITE ACCOUNT NUMBER of NATIONAL FREIGHT MGT . This will be given only to our CUSTOMERS
and is NOT to be used without our CONSENT.
- In the CHARGE TO box - SELECT the FREIGHT CHARGES by placing an X in the THIRD PARTY
box
- Full name & address ( including postcode ) of SENDER NO P.O BOX ADDRESSES
- Full NAME & ADDRESS ( including postcode ) of RECEIVER NO P.O BOX ADDRESSES
- SERVICE required is to be INDICATED by placing a X in the APPROPRIATE BOX If NO
box is ticked, the SHIPMENT will travel DOOR/DOOR
- DESCRIPTION of GOODS ( eg box, carton, skid, pallet )
- NUMBER of ITEM(S). This must match the number of items available for COLLECTION
- The DIMENSIONS of the ITEMS must be in METRES ( M )
- The WEIGHT of the ITEMS must be in TONS

How to Fill Consignment Note?
- SERVICE required is to be INDICATED by placing a X in the APPROPRIATE box. If NO
box is ticked, the SHIPMENT will be SENT and CHARGED at Company’s discretion.
- In the CHARGE TO BOX – fill in as – NATIONAL FREIGHT MGT WANTIRNA / VIC 3152
- Write ACCOUNT NUMBER of NATIONAL FREIGHT MGT. This will be given only to our CUSTOMERS
and is NOT to be used without our CONSENT.
- Full NAME & ADDRESS ( including postcode ) of SENDER NO P.O BOX ADDRESSES
- Full NAME & ADDRESS ( including postcode ) of RECEIVER NO P.O BOX ADDRESSES
- ONFORWARDING INSTRUCTIONS BOX –write SPECIFIC delivery time - if required.

- TNT – EXTENDED WARRANTY – must write the appropriate class – in the CLASS box either
A, B, or C. In the SUM REQUIRED box must write the VALUE of the GOODS to be INSURED

TNT INTERNATIONAL CONSIGNMENT
How to Fill Consignment Note?
- WRITE ACCOUNT NUMBER of NATIONAL FREIGHT MGT . This will be given only to our CUSTOMERS
and is NOT to be used without our CONSENT.
- Full name & address ( including postcode ) of SENDER NO P.O BOX ADDRESSES
- Full NAME & ADDRESS ( including postcode ) of RECEIVER NO P.O BOX ADDRESSES
- DANGEROUS GOODS declaration must be made fo every CONSIGNMENT If YES, a separate
SHIPPERS DECLARATION must be PROVIDED by the SENDER.
- All Consignments MUST be SIGNED & DATED. In doing so you are AGREEING to the terms
& conditions – which are on the back of every consignment note .
- SERVICE required is to be INDICATED by placing a X in the appropriate box. DIFFERENT
rates APPLY for each service. If NO box is marked & FREIGHT is under 30 kg, EXPRESS
service & RATE Applies. If OVER 30 kgs per ITEM, shipment travels as ECONOMY EXPRESS.
MULTIPLE item consignments where no INDIVIDUAL item EXCEEDS 30 kg May total up to
500 kgs on the EXPRESS service.

EXPRESS service is required – place an X or TICK the EXPRESS SERVICE BOX on the
right hand corner of the consignment note.

and is NOT to be used without our CONSENT. Write QUOTE number if one is given

- FULL DESCRIPTION OF GOODS. (e.g box, carton, skid, pallet ). IF SENDING ENGINES-
please write ENGINE TYPE ( e.g 4 cyl engine, 6 cyl motor, 8 cyl motor ) to be CHARGED
ENGINE RATE
- DANGEROUS GOODS – IF APPLICABLE
- CUSTOMER SIGNATURE & DATE. All Consignments must be SIGNED & DATED. In doing so
you are AGREEING to the terms & conditions – which are on the back of every consignment

HOW TO WORK OUT CHARGE WEIGHT
- ALL FREIGHT IS SUBJECT TO BE CHARGED BY DEAD OR CUBIC WEIGHT
- CHARGE WT = GREATER OF EITHER THE ACTUAL WT OR CUBIC WT OF AN ITEM
TO CALCULATE CUBIC CONVERSION:

- CUBIC CONVERSION FOR: EXP ROAD - 1 CUBIC METRE = 250 KG

INTERNATIONAL AIR - 1 CUBIC METRE = 167 KG
SEA FREIGHT - 1 CUBIC METRE = 1 TONNE ( 1000 KGS )
